What is ACTion (Movement) Initiatives?

ACTion (Movement) Intiaitves is Addressing Conditions to Improve Population Health. Utilizing the MKE SDOH ACTion Plan the ACTion Movement Initiatives Grant will focus on implementing solutions that address local policies, systemic gaps, and environmental factors that contribute to the health inequity in Milwaukee.


The ACTion Grant focuses on actions to impact SODH outcomes using a PSE approach, centering community voice, and public policy advocacy.


PSE Interventions

  • Enhancing Social Connectedness - Social connectedness relies on the space to

    share experiences. Through development of Community Health Action Teams (CHATs), hosting bi-annual SDOH Symposiums, and sponsoring or participating in Community Engagement Events, safe spaces for necessary social connectedness will be established.

  • Community - Clinical Linkages - Enhancing the Community-Clinical Linkages which

    includes optimizing connections and feedback loop between residents, health care providers, and community resource organizations. To maintain sustainability, The ACTion grant will contribute to sustaining the community - clinical connection through continued collaboration of community residents and clinical leaders.

  • Environmental Improvements - Community environmental actions based on walkability assessments and PhotoVoice submissions to carry out neighborhood cleanups, food waste reduction campaigns, and community gardening.
